This document describes how to get a Mobile Food Vendor License:
http://www.nyc.gov/html/doh/downloads/pdf/permit/mfv_license_appl.pdf
You’ll need a couple important things:
Some are easy:
- Completed Application
- Proof of Address
- SS Card
- Photo ID
- ECB Clearance
- Child Support Enforcement Form
- License Fee $50
Some are harder:
- NYS Sales Tax Certificate of Authority (this takes some time to come in the mail so do it first!)
- Food Protection Course
Here are some tips on the harder Items:
Sales Tax Certificate of Authority:
You can apply online for your NYS Certificate of Authority to Collect Sales Tax. Click Online Applications and select the Department of Taxation and Finance to file Form DTF-17, Application for a Sales Tax Certificate of Authority.

Food Protection Course:
Here are details on signing up for the food protection course for mobile food vendors.
Classroom Training $53, 2 sessions

If you have a card from the regular food protection course, or would rather take the longer course for restaurants, the restaurant food protection course counts towards this requirement. We recommend that everyone get the regular restaurant food protection course as it is more rigorous and it is more useful as it can be used to operate a Temporary Food Service Establishment (TFSE) or manage a restaurant:
Classroom Training: $114, 5 sessions

Online Training: $24.60

IMPORTANT – Make sure your name and address are 100% perfectly the same on every document before you go to apply for your license.
